Limitations

Due to space constraints, we are limited to 50 characters per line of text. Please fill out the fields below EXACTLY as your name should appear on your name tag. Please do not include credeintals, as we want all name tags to be consistent in style. Exception: If you have a doctorate degree, please indicate so in the prefix field.
